About the Program

The NCD Care Companion Program equips patients and their families with the knowledge and tools to effectively manage chronic diseases at home, reducing hospital visits and improving long-term health outcomes.

Challenge

Cultural and Linguistic Diversity: Designing content that is culturally relevant and accessible to users across different regions with varying languages and literacy levels.

Invisible Threats: The challenge had been addressing the silent progression of hypertension and diabetes, which often went undetected in younger adults due to the absence of noticeable symptoms. Efforts focused on promoting early screenings and preventive actions to mitigate the risk and long-term effects of these lifestyle diseases.

Scalability: Designing a system flexible enough to be adapted and implemented across diverse regions, each with unique challenges and healthcare environments.

Design Approach

The medical approach was rooted in evidence-based public health strategies, with a strong emphasis on accessibility, cultural relevance, and behavioral change. We engaged directly with patients, caregivers, and healthcare providers to understand the challenges of managing non-communicable diseases (NCDs) like hypertension and diabetes.

Through a co-creation process, we developed educational tools and intervention strategies, iteratively testing them to ensure they fit seamlessly into daily routines. By incorporating behavioral science principles, we encouraged long-term adherence to healthy practices, making these resources not just informative but transformative in improving chronic disease management.

To maximize reach and inclusivity, we utilized multi-channel health communication strategies:

- WhatsApp campaigns with rich media content provided interactive and engaging health education for smartphone users.
- Interactive Voice Response Systems (IVRS) ensured accessibility for non-smartphone users, delivering essential health information through phone calls.

These combined efforts made the program scalable, effective, and adaptable to different community health settings.

My Role

As a Public Health Content Manager for Non-Communicable Diseases, I led the development and implementation of educational materials that enhanced chronic disease management. I collaborated with healthcare professionals, patients, and behavioral scientists to ensure that the content was medically accurate, culturally relevant, and actionable.

My role also involved:
- Designing and refining NCD-focused health communication tools to improve patient and caregiver engagement.
- Testing and optimizing interventions for real-world application and effectiveness.
- Implementing scalable content strategies to ensure widespread adoption across diverse communities.

Through these efforts, I worked toward empowering individuals and healthcare providers with the knowledge and tools needed to manage non-communicable diseases effectively.
